• Initial pool fund for Hospitalization Assistance: Php 700.00

OR you may give an Initial Share Cap minimum of Php 5 000.00. The minimum share capital (Php 20 000.00) shall paid within 3 years.

Associate Account Members

Laboratory Account Members

• 0 - 23 years old

• Laboratory/Kiddie Membership Form
• 2 PCS 1X1 ID picture of the Child
• 2 PCS 1X1 ID picture of the Guardian
• 1 Photocopy of Valid ID of Guardian
• The Guardian must have an account either Regular or Associate
• Preferred Share: Php 1 000.00
• Opening/Maintaining Balance: Php 500.00
• Membership fee: Php 25.00
• Passbook fee: Php 50.00
